Testing and Applying Chap Stick

Madison is in the process of getting her re-evaluation testing done to see whether or not she still needs Speech and Occupational Therapy through the school system. Today I received the School Psychologist's report in the mail. Here are some highlights from the report:

Teacher Interview:
Madison is a leader in the classroom and displays well developed skills in all areas. Madison has adjusted to the classroom routine and gets along well with her peers. She is very expressive and displays above average vocabulary and language skills. Madison does not like certain textures on her hands but she does always participate in hands on activities. She becomes very upset if mildly reprimanded.

Behavioral Observations:
Madison willingly accompanied the examiner to the testing area. She was cooperative and attempted all tasks. Madison was very friendly and eager to participate in the testing procedures. She displayed a well developed attention span and followed directions easily. Madison displayed above average language skills and was very comfortable interacting with the examiner. At one point, she pulled out a small mirror and began applying chap stick.

Summary:
Madison scored within the high average range of overall intellectual ability and academic achievement. She displayed appropriate interaction skills and was very comfortable in the testing environment.

IEP Meeting

Today we had our annual IEP (Individual Education Plan) meeting with Madison's teacher and her Speech and OT therapists. We discussed Madison's progress and wrote new goals for her for the next school year.

Behavioral
Measurable Annual Goal/Expected Outcome
Madison will act in an age appropriate manner when she is disappointed, doesn't get her way, and play in groups of 3 or more girls 10/10 consecutive days observed.

Benchmarks/Short Term Objectives
Madison will play for 20-30 minutes with 3 or more girls with no disagreements and hurt feelings by December 2009.
If Madison does not get her first pick of a job, task, or turn she will not pout or cry, by May 2010.

Academic
Measurable Annual Goal/Expected Outcome
Madison will increase her academic skills to an age appropriate level for math, reading, and writing skills with recognizing numbers to 20, identifying letter sounds, and writing name and letters 100% by the end of the IEP period.

Benchmarks/Short Term Objectives:
Madison will count to 15 and recognize numbers in print by December 2009.
Madison will count to 20 and recognize numbers in print by May 2010.
Madison will be able to give the sound letter of the alphabet by December 2009.
Madison will be able to write her numbers to 10 and her first name by December 2009.
Madison will be able to write her first and last name and all letters upper and lowercase of the alphabet by May 2010.

Occupational Therapy
Measurable Annual Goal/Expected Outcome
Madison will complete fine/visual motor skills with visual model 4/5 trials to increase independence with classroom tasks.
Benchmarks/Short Term Objectives
Madison will trace shapes and connect dot to dot pictures with good directionality.
Madison will cut straight and curved lines of a picture to produce recognizable shapes.
Madison will complete figure ground and mazes with good scanning skills.

Speech Therapy
Measurable Annual Goal/Expected Outcome
Madison will correctly produce the phonemes /sh/ and /ch/ in all positions of words in connected sentences with 80% reliability.
Benchmarks/Short Term Objectives
Madison will correctly produce the target phonemes in all positions of words in 8/10 trails across three therapy sessions.
Madison will correctly produce the target phonemes in all positions of words in elicited phrases in 8/10 trials across three therapy sessions.
Madison will correctly produce the target phonemes in all positions of words in elicited sentences in 8/10 trials across three therapy sessions.
Madison will correctly produce the target phonemes in all positions of words in 2-3 elicited connected sentences in 8/10 trials across three therapy sessions.

Individual Education Plan (IEP)

After extensive testing through the Okaloosa County School System, as well as, private consultants, it was determined that Madison qualified for an Individual Education Plan (IEP). She is eligible based on three categories: DD-Developmental Delay, OT-Occupational Therapy, and SPI-Speech Impaired.

DD
Measurable Annual Developmental Delay Goal:
Madison will comply with adult directives or requests without resistance i.e. tantrums and screaming, in 4/5 observation by the end of the IEP period.
DD Benchmarks/Short Term Objectives:
1. Madison will follow the classroom routine (line up, get materials, complete a task) with visual and verbal cues 4/5 times per day by the end of the IEP period.
2. Madison will comply with an adult request/directive without a physical tantrum 4/5 times/day by the end of the IEP period.
3. Madison will comply with an adult request/directive without verbal protest i.e. screaming 4/5 times/day by the end of the IEP period.
SOCIAL SKILLS
Measurable Annual Social Skill Goal:
Madison will play interactively with 2 or more classmates for 10 minutes on 4/5 observations by the end of the IEP period.
Social Skill Benchmarks/Short Term Objectives:
1. Madison will respond to appropriately to the attempts of her peers to engage her in a play interaction 3/5 times by the end of the IEP period.
2. Madison will physically approach and engage classmates to play interactively at lest 3 times a day by the end of the IEP period.
3. Madison will appropriately initiate greetings to others 4/5 times per week by the end of the IEP period.
4. Madison will appropriate respond to greetings from others at least 4/5 times per week by the end of the IEP period.
OT
Measurable Annual Occupational Therapy Goal:
Madison will demonstrate improved fine motor, self-help and sensory processing skills by accurately tracing uppercase letters, completing clothing fasteners, and by tolerating uncontrollable sensory experiences in the educational environment, 3/4 trials.
OT Benchmarks/Short Term Objectives:
1. Madison will trace simple shapes (circle, square, triangle) accurately, 2/3 trials.
2. Madison will participate in cut and glue activities, given 2 opportunities to wash hands in a 15 minute period, 2/4 trials.
3. Madison will utilize a sensory die of calming/transitioning activities (picture schedule, headphones, etc.), to tolerate the educational setting and routine, 3/4 trials.
Madison will be receiving OT once a week for a 30 minute session at Longwood Elementary with the school occupational therapist. I will also be taking her once a week to a private occupational therapist at the Pediatric Therapy Center.
SPEECH
Measurable Annual Speech Goal:
Madison will correctly articulate age appropriate phonemes (b, p, m, g, k, f, and v) in a five minute conversational sample with 80% accuracy.
Speech Benchmarks/Short Term Objectives:
1. Madison will use age appropriate phonemes at the end of words in structured sentences.
2. Madison will correctly pronounce /f/ and /v/ in structured speaking.
Madison will be receiving speech therapy twice a week for 30 minutes a session at Longwood Elementary with the school speech therapist. I will also be taking her once a week to a private speech therapist at Pediatric Therapy Center.
